BREAST RECONSTRUCTION--TRAM FLAP  
 
 

Problem.

Loss of breast tissue and skin following a mastectomy.

Who is a candidate?

  • Anyone who has lost breast tissue and skin due to a mastectomy on one or both sides.
     
  • A person with a loss of breast tissue following a subcutaneous mastectomy.

Procedure description.

With a TRAM Flap procedure, Dr. Casso transfers muscle and fat from the lower abdominal wall to the area of the missing breast tissue. Blood supply to this tissue is within the rectus muscle which is left attached at the lower edge of the rib cage. The abdominal site is closed in the same manner as a Tummy Tuck leaving a horizontal scar and a tighter abdomen. Dr. Casso creates a breast shape during the first procedure, but oftentimes subsequent smaller procedures are necessary for optimal breast shaping.

Recuperation and healing.

All of the TRAM Flap operations require a stay of several days at CHRISTUS ST. JOHN HOSPITAL. The most discomfort will come from the abdominal site. Walking begins the day after surgery and will be comfortable within 10 to 14 days. Complete recovery takes about six weeks.

Complementary procedures.

Procedures that can enhance the results are enlargement, lifting or reduction of the opposite breast, and nipple-areola reconstruction can be performed.

The specific risks and the suitability of this procedure for a given individual can be determined only at the time of consultation. All surgical procedures have some degree of risk. Minor complications that do not affect the outcome occur occasionally. Major complications are rare.
